When does it happen? Right after a mag change? At the end of a mag? It might be your grip. When you change mags, you don't get right back into your best grip position. Sometimes it takes a shot to get it right. How many of us have "thrown" the first shot of a mag? If it's at the end of a mag, then it might be fatigue. Rest the gun halfway through the mag. I find that when I change out mags that I have to really think about that first shot and my grip. I will put a little extra tightness on the fingers.
